I. Description of Smoke-Free Family

  1. Smoke-Free Family is a program that invites all Quebec families to eliminate second-hand smoke from their living environments. The program also strives to support these families as they declare their surroundings smoke-free.
  2. Smoke-Free Family invites all Quebec families to become a Smoke-Free Family. For the purposes of this program, the word “family” designates a group of people living in the same environment (principal area of residence). It can also designate a person living alone.
  3. For participating families, Smoke-Free Family consists of committing to having no one smoke inside the house/apartment and car, and officially declaring their living environments smoke-free. Families can register starting January 1, 2010 until March 31, 2010.
  4. Registration for Smoke-Free Family is free of charge and all families registered may modify their registration file at any time by going to TakeYourButtOutside.ca

II. Eligibility

  1. All families may participate in Smoke-Free Family, whether or not family members smoke. A family representative speaks for all family members and, among other things, registers the family and provides family members’ information.

    To be eligible for the Smoke-Free Family program, the participating family representatives must:

    • be duly registered, along with their family, for Smoke-Free Family;
    • commit to having no one smoke inside their house/apartment and car, and officially declare their living environment smoke-free.

III. Registration

  1. Registration for Smoke-Free Family takes place from January 1, 2010 at noon [ET] to March 31, 2010 at 11:59 PM [ET].
  2. The representative for the family interested in participating in Smoke-Free Family must fill out and send in A SINGLE REGISTRATION FORM for Smoke-Free Family: either a paper registration form, duly filled out, signed, and sent by mail in an envelope with sufficient postage, or by using the online registration form on the Smoke-Free Family website. All registration form can be submitted at all time between January 1, 2010 to March 31, 2010. A single registration form per home will be accepted.
  3. The registration form must be sent by one of the following means:
    • by Internet: Fill out and submit the online registration form on the Smoke-Free Family website at TakeYourButtOutside.ca
    • by mail:

  4. Family representatives must provide their contact information on the registration form and answer questions about their family. Each representative will receive a registration confirmation (immediately on the website if registering online, and by mail if registering using the paper form).
  5. By submitting the Smoke-Free Family registration form online at TakeYourButtOutside.ca or by mail, family representatives declare that they have read and understood the Smoke-Free Family rules, that they satisfy their conditions and are committed to respecting them.
  6. By submitting the Smoke-Free Family registration form either online at TakeYourButtOutside.ca or by mail, family representatives confirm their name, age, residential address, and telephone numbers where they may be reached, as well as contact information for other family members. Several questions are also asked for statistical purposes.
  7. By submitting the Smoke-Free Family registration form online at TakeYourButtOutside.ca or by mail, family representatives commit to having no one smoke inside their house/apartment and car and officially declare their living environment smoke-free.

IV. Authority and Responsibilities

  1. ACTI-MENU holds FINAL AUTHORITY in matters of judgment and decisions regarding the eligibility of families to be registered for Smoke-Free Family.

V. ACTI-MENU’s policy for the protection of personal information

Policy for the protection of personal information as applied by ACTI-MENU, its employees and representatives.

  1. Introduction
    In its normal course of business, ACTI-MENU collects personal information about its employees, its clients and their employees, participants in its various campaigns and its subcontractors. All personal information collected respects the general laws and regulations that apply to confidentiality and/or is subject to ACTI-MENU’s policy for the protection of personal information.

    ACTI-MENU is committed to ensuring the accuracy, confidentiality, security and private nature of the personal information it collects by stringently applying its policy and ensuring that it is respected by all of its employees and representatives.

  2. Responsibility
    The directors of business units and departments are responsible for ensuring that the policy is respected and taking the appropriate measures to facilitate its application on a day-to-day basis. In cases where the policy is not respected, they will take appropriate measures to correct the situation.
  3. Determining how information collected will be used
    At the time it collects personal information, ACTI-MENU will specify in a document or an agreement how the information will be used and will provide written notification to the people from whom the information is being collected.

    The collection of information is limited to the purposes for which it was authorized. Otherwise, ACTI-MENU will obtain additional consent as required.

  4. Consent
    ACTI-MENU will inform and obtain consent from all persons concerned before it collects, uses or transmits personal information, unless it is inappropriate to do so under applicable laws.
  5. Limits on information collected
    ACTI-MENU will take measures to ensure that only information needed for the purpose identified will be collected. The personal information will not be transmitted or used for any other purpose other than that for which consent was obtained.

    Subject to applicable laws, ACTI-MENU may keep personal information on file for as long as necessary for pre-established purposes, even if the person is no longer an employee, client or participant.

  6. Accuracy
    ACTI-MENU’s employees are required to inform the directors of business units or departments of any changes to information in their file. Clients, participants and subcontractors are also requested to provide any changes to information previously submitted to ACTI-MENU.

    ACTI-MENU will update personal information as soon as it is received from employees, clients, participants and subcontractors, when it is relevant to do so.

  7. Security
    Personal information is protected by security measures that are commensurate with the degree of sensitivity of the information.

    Security measures protect personal information against loss or theft and against unauthorized consultation, transmission, copying, use or modification.

    The nature of the security measures will vary depending on the degree of sensitivity of the personal information, the quantity, distribution and format of the information, as well as how it is conserved. Various methods may be used, such as rendering the information anonymous, restricting access, applying password protection or any other means judged to be appropriate under the circumstances.

  8. Transparency
    This policy is distributed to all managers, administrators, employees and contributors and to any person who requests it.
  9. Access to personal information
    ACTI-MENU will inform anyone who inquires as to the existence of personal information how the information is used and the fact that it has been provided to third parties, and will allow them to consult it. It will be possible to confirm the accuracy and comprehensiveness of the information and to make corrections if required. Complaints will be handled according to the appropriate procedures and within a reasonable timeframe. This policy will be upgraded periodically.
